PCC believes that primary care exists to foster patients’ personal well-being. The best models of care are person-centered and build from ongoing relationships between individuals and their care teams. These models put patients are at the center of their own care, with decisions made jointly and driven by individuals’ own needs, preferences, and social context.   

We work with patients, families, and advocates to ensure that all advanced primary care models incorporate patients' perspectives, needs, and experiences.

Engaging Patient & Family Practice Partners in Practice Transformation

One example of this approach is PCC’s TCPI work. As a Support and Alignment Network (SAN) in the Transforming Clinical Practices Initiative (TCPI), PCC provided support to patient and family partners, along with practices and Practice Transformation Networks across the U.S., to promote more meaningful patient partnerships in quality improvement and community collaboration with care teams. Our goal was to help participating clinicians partner with patients and families to achieve TCPI’s phases of transformation and associated milestones, resulting in improved care, better health, and reduced costs.
