Care Coordination and the Patient's Role in Shared Decision Making and Team Communication

 In this webinar, we explore the definition of the care team and care coordination as well as the key elements of care coordination within the PCMH. We will also talk about the patient’s perspective by reviewing Christine Bechtel’s research on patients and the delivery system as a whole – its challenges and potential solutions – including care coordination and the medical home.

Christine Bechtel is a longtime consumer advocate and the co-founder of X4 Health, a purpose-driven organization that uses social impact design to solve problems that matter. 3rd Conversation is one of X4’s programs. Christine has been involved with PCC since its earliest days, having co-chaired a number of PCMH-related working groups. She helped facilitate the development of the Shared Principles of Primary Care across 300 stakeholders on behalf of the PCC. She also spent 18 months working with a primary care practice where she helped its leaders establish a culture of patient- and family-centered care and professional satisfaction. Vice President, Health Care Delivery System Reform Program, The Commonwealth Fund
Melinda K. Abrams, M.S., is Vice President of the Commonwealth Fund's Health Care Delivery System Reform Program. Since joining the Commonwealth Fund in 1997, Ms. Abrams has worked on the Task Force on Academic Health Centers, Commission on Women's Health, and most recently, the Child Development and Preventive Care programs. She played a lead role in conceptualizing and launching the Fund's Assuring Better Child Health and Development (ABCD) initiative, which awarded grants to state Medicaid programs to encourage innovation in the financing and delivery of preventive and developmental services provided to low-income, young children.
