For a more advanced search of programs, you can view by location (National View) or by program name (List View).
The National Map View allows users to find programs by location. Programs are mapped by location of principle staff or program headquarters. Statewide programs are mapped to the state capitol. Users can use many criteria to filter the map.
The List Map View allows users to obtain a list of programs based on selected criteria. This view shows program name, location and types of outcomes that have been publicly reported (i.e. cost savings, improved satisfaction, utilization, etc.).
The Outcomes Map View allows users to access peer-reviewed and industry-reported evaluation data for programs featured in the map. Users can search programs by type of outcome (e.g., cost savings, fewer ED visits, improved health, etc.).
Our Map Tools page includes two customizable slide presentations. Slide Deck 1: PCMH 101 - General Overview. Slide Deck 2: PCMH 201 - A Snapshot of the Evidence.
