Activating Patient Engagement in Care Delivery: Performance Metrics that Guide Patient-Centered Care

Practices participating in the Transforming Clinical Practice Initiative (TCPI) are constantly aiming towards effective transformation through the implementation of meaningful person-centered care. In support of this goal, TCPI recently released Person and Family Engagement (PFE) performance measures that healthcare staff can use to strengthen partnerships in ambulatory care. 

This webinar is part one of a two-part series that will provide attendees with an overview of TCPI's Person and Family Engagement (PFE) performance metrics. In collaboration with our partners at the Institute for Patient and Family-Centered Care, we will describe shared decision making, patient activation, and health literacy -- three of the six PFE metrics-- and explore examples where effective partnerships with patients in direct care were achieved and sustained. Success stories illuminating how these strategies were implemented as well as the targeted resources/tools available will also be shared. 

Note: The examples and tools are applicable to any practice interested in preparing for the CMS Quality Payment Program!
