About Us
The Primary Care Collaborative (PCC) is the leading national, non-partisan and multi-stakeholder voice advocating for better health and wellbeing for all Americans by strengthening primary care. The PCC unifies and engages diverse stakeholders in promoting payment/delivery policies and sharing best practices that encourage the growth of team-based, comprehensive primary care.
Founded in 2006, the PCC represents a broad coalition that includes: patient and consumer groups; primary care practices; clinician specialty societies; payers and insurers; employers and purchasers; technology and pharmaceutical companies; and other health care organizations.
Mission
The Primary Care Collaborative advances comprehensive primary care to improve health and health care for patients and their families by convening and uniting stakeholders around research, care delivery and payment models, and policies.

Members
The Primary Care Collaborative's membership (Executive Members) reflects all the different stakeholders in health care and is the source of the organization's effectiveness. The PCC brings together physician and nursing groups, patient and consumer organizations, health plans, employers, industry, quality organizations, health systems and many more to forge consensus around policy solutions that further primary care. Below you will find information and education relevant to all these stakeholders.
Activities
Disseminate
Disseminate results and outcomes from advanced primary care and medical home initiatives and clearly communicate their impact on patient experience, quality of care, population health and health care costs.
Educate
Educate stakeholders and advocate for public policy that advances and builds support for primary care and the medical home, including payment reform, patient engagement, and employer benefit initiatives.
Convene
Convene health care experts, patients and thought leaders in the private and public sectors to promote learning, awareness, and innovation of the medical home model.
