Linking Patient-Centered Medical Home and Asthma Measures Reduces Hospital Readmission Rates

OBJECTIVE: We sought to achieve 100% compliance with all 3 Children’s Asthma Care (CAC; CAC-1, CAC-2, CAC-3) measures and track attendance at follow-up appointments with the patient-centered medical home. The impact of these measures on readmission and emergency department utilization rates was evaluated.

Every state’s Medicaid and CHIP program is changing and improving – most states are expanding coverage for low-income adults; all states are modernizing their Medicaid/CHIP eligibility, enrollment and renewal processes and systems, and taking advantage of many of the new flexibilities provided by the Affordable Care Act.

Peer Support in Health Evidence to Action

Major reviews document that peer support provided by “community health workers”, “promotores”, etc. make important contributions to health, health care and prevention (Viswanathan et al. Med Care 2010 48, 792-808; Gibbons et al. Prog Community Health Partnersh 2007 1, 371-381; Swider. Public Health Nurs 2002 19, 11-20; Perry et al. Annu Rev Public Health 2014 35).In diabetes management, 19 of 20 articles published between 2000 and 2012 showed signifi cant evidence of benefits of peer support.

What We’re Learning: Engaging Patients Improves Health and Health Care

People who are actively engaged in their health care are more likely to stay healthy and manage their conditions by asking their doctors questions about their care, following treatment plans, eating right, exercising, and receiving health screenings and immunizations. Patients without the skills to manage their health care incur costs up to 21 percent higher than patients who are highly engaged in their care.

Health Literacy Universal Precautions Toolkit


The Agency for Healthcare Research and Quality commissioned the University of North Carolina at Chapel Hill to develop and test this Health Literacy Universal Precautions Toolkit. The toolkit offers primary care practices a way to assess their services for health literacy considerations, raise awareness of the entire staff, and work on specific areas.

Strategies that address major CVD risk factors, such as high blood pressure, high cholesterol, and smoking, can greatly reduce the burden of CVD. 'Activated' Patients Have Better Care Experiences: New Study

Abstract

Patients who have the knowledge, skills, and confidence to manage their health care report better health care experiences than less "activated" patients seeing the same providers, a new Health Affairs study finds.
