Collaboration and Practice Transformation in Residency Education

The current fee-for-service model of primary care delivery in the United States is outdated and wreaking havoc on the foundation of our healthcare system. This model will be phased out over the next decade in favor of value-based reimbursement (among other changes). Residency programs, however, are often based on the old model of care and, consequently, are not prepared to train residents for the future models of care. In order to address this deficiency, residency-training collaboratives have been popping up around the country.

Primary Care, ACOs, and Payment Reform: A Conversation with Mark McClellan of the Brookings Institution

 

Mark McClellan of the Brookings Institution joined a PCPCC webinar to discuss the important role primary care leadership plays in health care transformation, including physician-led ACOs and other payment reform initiatives. 

He highlighted major themes and recommendations from a recently released ACO Learning Network toolkit, Adopting Accountable Care: An Implementation Guide for Physician Practices. The four major themes include:

PCPCC Chief Executive Officer, Marci Nielsen, provides an overview of the PCPCC's latest evidence report titled, The Patient-Centered Medical Home's Impact on Cost and Quality: Review of Evidence 2013-2014. The publication highlights evidence from primary care PCMH initiatives taking place in both public and private markets across the country. The report looks at selected outcomes from 28 peer-reviewed studies, state government evaluations, and industry reports published between September 2013 and November 2014.

Interprofessional Primary Care Training: Seven Champion Programs

Marci Nielsen, PhD, MPH and Barbara Brandt, PhD discuss a new PCPCC publication that focuses on interprofessional primary care training. The publication takes a deeper dive into seven exemplary programs that train medical students, residents, nurse practitioners, nurses, physician assistants, social workers, psychologists, and other health professionals for work in patient-centered medical homes.
