This peer-reviewed report on health care transition for pediatric to adult health care includes a process for transition preparation, planning, tracking, and follow-through and is based on more than two decades of evidence-based research. A special focus is placed on young adults with special health care needs (SHCN) and families.

Greater use of primary care is associated with lower costs, higher patient satisfaction, fewer hospitalizations and emergency department visits, and lower mortality. Despite current high levels of healthcare spending in the United States, the proportion spent on primary care is insufficient. A shift in resources to support greater access to comprehensive, coordinated primary care is imperative to achieving a stronger, higher-performing healthcare system.

New toolkit to Prepare Patient Stakeholders to Work with Research Teams

The toolkit contains educational materials and training templates for effectively engaging patient advisors in research projects. Materials can be tailored for each research project. 

 

Latest PCMH Results and Learnings from 7 Years

The latest results are in. CareFirst’s Patient-Centered Medical Home (PCMH) program – the largest and most established program of its type – is driving improvements in care and cost through patient-centered care. Members enrolled in the program are benefitting through:

• Fewer hospital admissions and readmissions
• Fewer days in the hospital
• Lower cost per emergency room visit

The program has also slowed cost growth by more than a billion dollars against the expected cost of care in the company’s Maryland, DC and Virginia service area.

TCPI Medication Management and Opioid (MMO) Initiative Pledge

TCPI launched the Medication Management and Opioid Initiative in alignment with the TCPI 7 Aims. The intent is to generate commitments from TCPI and partner clinicians to be in action by signing the MMO Pledge. This pledge will be on a public page of www.healthcarecommunities.org and will be open to TCPI-enrolled clinicians to complete. This resource is posted for information on the Pledge.  Clinicians who are interested in participating should visit the www.healthcarecommunities.org page.

The Impact of Primary Care Practice Transformation on Cost, Quality, and Utilization reviews PCMH results from 45 peer-reviewed reports and additional government and state evaluations. Implementation of the PCMH and high performing primary care differs depending upon the needs and preferences of those delivering, receiving, and paying for related care. 

Evaluation of the State of Minnesota's Health Care Homes Initiative

This report to the Minnesota Legislature is an evaluation of the efficacy of the Health Care Homes Initiative. It describes differences between certified Health Care Homes clinics (HCH) and those clinics that, though eligible, choose not to be certified (non-HCH). Using Medicare and Medicaid claims data, this report addresses differences in cost, utilization, and disparities between Health Care Homes and nonHealth Care Homes from 2010 through 2014.
