May Workshop - How to Make the Business Case for Person-Centered Care

This presentation will define and provide a framework for establishing the business case for PCC.  Considerable interest has been shown recently in person-centered care (PCC) models of health care delivery.  In PCC, the goals of care are determined or at least significantly shaped by the individual and family and the care plan is reflective of their goals and values.

Interest in PCC has been based largely on its potential to improve both the experience for the person and health outcomes, including quality of life.  Particular interest has been shown in PCC for older adults with multiple chronic conditions and functional limitations, a population characterized by high levels of medical utilization and expense.  To implement PCC, often times organizations are required to present a business case to elicit the necessary buy-in from Leadership and the Board.
