The PCPCC Support and Alignment Network (SAN) is providing technical assistance and sharing best practices to promote meaningful patient partnerships in quality improvement and community collaboration. Get involved today! 
Is Your Practice Partnering with Patients in Quality Improvement? 
 
The PCPCC SAN is developing a new online map (coming soon) that will identify where and how practices are partnering with patients in practice transformation! We understand that these partnerships often vary by practice and can include a variety of innovative activity from convening Patient and Family Advisory Councils, to hiring patients or caregivers as professionals on the care team, to involving patients in the governance of the practice/organization, etc.

Join a Virtual Community of Patient and Family Advisors
 
The PCPCC is collaborating with the Institute for Patient- and Family-Centered Care (IPFCC) to connect patient and family partners in IPFCC's online PFACNetwork. By joining this online community, practices and patient partners will receive information about upcoming trainings (virtual and in-person events), links to tools and resources, and opportunities to communicate with other patients and family caregivers doing this work nationwide.

Assessing the Practice in Partnership with Patients & Families - Opportunities to Improve Patient- and Family-Centered Care

Redesigning your clinical practice can be overwhelming. Where do you start? How do you prioritize your quality improvement efforts? Patients and family caregivers can provide crucial insight that will help you make these decisions. Patients usually know what is working for other patients in the practice and what changes could make a big difference in making the practice more patient- and family-centered. Sometimes the greatest impacts on improving care through the eyes of the patient only requires modest effort by the practice.

*Immediately following the 60-minute webinar, there will be an optional 30-minute Q&A session with the presenters.
