Dear Members and Friends:

We're delighted to be at the Fifth National Medical Home Summit with so many of you in Philadelphia this week, and for that reason we are taking a break from our usual Week in Review format.

Today we are thrilled to announce that the PCPCC's 2013 Annual Fall Conference Community Connections in the Medical Neighborhood: The Future of the Primary Care Medical Home will focus on the medical home's key foundational role within the larger health care system. This trend is encouraging stakeholders across the care continuum to embrace community-based collaborations as a strategy to manage population health and create accountable, integrated systems of care.

Today marks the official launch of online registration, where you can find an agenda filled with experts and leaders on timely topics, such as Accountable Care Organizations (ACOs), state health reform, payment reform, education and training, population health, and patient experience.
