The Idaho Medical Home Collaborative (IMHC) is a collaboration of primary care physicians, private health insurers, healthcare organizations, and Idaho Medicaid created to make recommendations to the governor on the development, promotion, and implementation of a patient-centered medical home (PCMH) model of care statewide. The IMHC officially commenced on January 1, 2013. Practices receive technical assistance in the form of materials, coaching, and technical assistance webinars. The pilot will be evaluated using nationally-recognized measures of quality, efficiency, and patient-centered care. Primary care practices selected for participation in the IMHC were required to receive at least level 1 NCQA recognition by the end of year two of participation. Practices are required to report clinical quality data including 2 clinical quality measures, 2 preventive quality measures and 2 practice transformation measures. Four health plans are participating in the program and have agreed to provide enhanced PMPM.
The Idaho Medical Home Collaborative (IMHC) pilot will end in December of 2014 and will transition into the Idaho Health Care Coalition. It will maintain the same infrastructure and basic operations and "the private insurers involved in the pilot have pledged to continue participating in the medical home venture, and practices will begin being paid according to an incentive-based system." - AAFP News
