Under the the Cigna Accountable Care Program, HealthCare Partners doctors monitor and coordinate all aspects of an individual's medical care. Patients continue to go to their current physician and care team and benefit from preventive care programs and disease management programs. Patients most likely to see the immediate benefits of the program are those patients actively engaged with their care teams and those who need help managing chronic conditions, such as diabetes or heart disease. Critical to the program's benefits is HealthCare Partners coordinated care model and a wide array of Cigna's resources. The HealthCare Partners' care coordinators are aligned to a team of Cigna case managers to ensure a high degree of collaboration. The care coordinators and Cigna case managers can also refer individuals to any Cigna clinical programs available through their employee benefits plan, such as disease management programs for diabetes, heart disease and other conditions or lifestyle management programs, such as programs for tobacco cessation, weight management and stress management. Together the collaboration will reduce potential gaps in care such as patients failing to refill prescription drugs or patients overdue for health screenings and immunizations.
Cigna will compensate for the medical and care coordination services it provides. Additionally, the medical group may be rewarded through a “pay for value” structure if it meets targets for improving quality and lowering medical costs.
