The PA Center for Practice Transformation and Innovation evolved from the initial PA Chronic Care Initiative within the PA Department of Health. The PA Center for Practice Transformation and Innovation is a public/private partnership that includes insurers from all regions of the Commonwealth, provider organizations, health systems, educational institutions and government agencies. The specific work for this group included chronic care, patient centered medical homes, practice transformation, quality improvement.
We are a public 501(c)(3) community mental health center located in Williamsburg, Virginia. We provide a broad continuum of services to persons with mental health issues, as well as intellectual/developmental disabilities and substance use disorders. We also partner with multiple primary healthcare providers to integrate behavioral health and primary care services.
Children's Mercy Pediatric Care Network (CMPCN) is an integrated pediatric network that coordinates the medical care of pediatric patients.
CMPCN is comprised of Children's Mercy Hospitals and Clinics, community pediatricians, and other health care providers in the Kansas City area.
CMPCN uses a team-based approach so that the focus can truly be on the right care, at the right time, in the right setting.
Johns Hopkins Community Physicians is a large physician group owned and affiliated by Johns Hopkins Health System. We have 36 sites all over the state of Maryland. We participate in a number of PCMH program, including a state wide multi-paer demonstration program. Five of our sites are currently recognize by the NCQA at level 3. Five additional sites have applications in process.
