Our mission is to work under the patient and family centered care model along with evidence based principles The internal medicine clinic provides care to approximately 12,000 enrolled beneficiaries and generates approximately 36,000 outpatient visits per year.
The Community Clinic serves as a primary care clinic with pediatrics, obstetrics, gynecology, and adult care services available. Care is provided by family physicians. It is the only sliding scale clinic in the county. It is a primary care clinic and is seen as the safety net clinic in the county. The clinic is an integral part of the Holmes County General Health District.

Cherokee Health has administered a primary behavioral health integrated care practice model for over 30 years. We now provide training, consultation and technical assistance to other organizations around the country on the implementation of an integrated practice model.
