Medication Management is an issue that impacts health outcomes and safety. Literature on the topic highlights the disconnect that can occur between the healthcare team and the patient goals. For instance, patients do not take their medicine as prescribed 50% of the time. This number may be as high as 80% for patients with many chronic illnesses, especially for those who are asymptomatic. Over 25% of initial prescriptions are never filled. For diabetes care, this may be as high as 40%.
