Individuals with disabilities dually eligible for Medicare and Medicaid are at higher risk for poor health outcomes when compared to individuals without disabilities.[1] This conversation, in collaboration with ICS, will provide a platform for questions to be answered on how health plans and providers can improve health outcomes for this population by addressing gaps in care coordination that often disrupt continuity of care and lead to fragmentation of care.

The promise of using data to make more informed decisions seems simple. Consumers make data-driven decisions every day when buying household goods, booking hotels, or choosing where to eat. Businesses use data every day to determine the ads we see, the best location for expansion, or the cost of their services. But using data to drive decisions about social programs continues to remain out of reach for many policymakers and program administrators at all levels.
The State of New Jersey is saving $1.5 billion on prescription drug costs for its unionized employees over the next three years and health costs for members of Nebraska’s food workers union are down 5 to 15 percent. However, it will ultimately save time, lower costs, and most importantly, provide patients with the best, most appropriate care.
