Carolyn Clevenger, RN, DNP, is a gerontological nurse practitioner and is the Associate Dean for Clinical and Community Partnerships at the Nell Hodgson Woodruff School of Nursing. She is responsible for the Office of Clinical Placements and the Emory Nursing Professional Development Center including the Wound Ostomy Continence Nursing Education Center. Her nurse-led patient centered medical home for people living with dementia, the Integrated Memory Care Clinic, represents a one-of-a-kind clinical model that provides integrated memory, palliative, and primary care.
