Disability-Competent Care Conversation On Care Coordination With ICS

Individuals with disabilities dually eligible for Medicare and Medicaid are at higher risk for poor health outcomes when compared to individuals without disabilities.[1]  This conversation, in collaboration with ICS, will provide a platform for questions to be answered on how health plans and providers can improve health outcomes for this population by addressing gaps in care coordination that often disrupt continuity of care and lead to fragmentation of care.

This conversation will focus on the key elements of and strategies to improve care coordination, including the role of interdisciplinary teams. Representatives from ICS will answer your care coordination questions. We encourage attendees to submit questions for ICS ahead of time using any of the three options below:

Intended Audience:

This conversation is intended for a wide range of stakeholders - health plans, providers, and other health care professionals who serve individuals dually eligible for Medicare and Medicaid - to address gaps in coordinating care for individuals with disabilities.
