[Ended October 31st, 2014] The Federally Qualified Health Center (FQHC) Advanced Primary Care Practice demonstration will show how the patient-centered medical home model can improve quality of care, promote better health, and lower costs. Conducted in all states except DL, NV, UT, and VT by July, 2013 The 3-year Demonstration is designed to evaluate the effect of the advanced primary care practice model, commonly referred to as the patient-centered medical home, in improving care, promoting health, and reducing the cost of care provided to Medicare beneficiaries served by FQHCs. This Demonstration will be conducted in accordance with the Secretary’s demonstration authority under section 1115A of the Social Security Act. Participating FQHCs are expected to achieve Level 3 patient-centered medical home recognition, help patients manage chronic conditions, as well as actively coordinate care for patients.
PCPCC's PCMH map does not include individual links to the 434 participating sites, but an interactive map of these practices can be found by clicking on the CMS program website below.
To help participating FQHCs make these investments in patient care and infrastructure, they will be paid a monthly care management fee for each eligible Medicare beneficiary receiving primary care services. A monthly care management fee of $6.00 for each eligible Medicare beneficiary attributed to their practice to help defray the cost of transformation into a person-centered, coordinated, seamless primary care practice. This payment, which will be made quarterly, is in addition to the usual all-inclusive payment FQHCs receive for providing Medicare covered services. CMS and HRSA will provide technical assistance to help FQHCs achieve these goals.
