The state of New York receivied a Pre-Testing Award from the Centers for Medicare & Medicaid Services to further develop and refine its comprehensive State Health Care Innovation Plan. New York’s State Health Innovation Plan serves as a roadmap to achieve the “Triple Aim” for all New Yorkers: improved health, better health care quality and consumer experience, and lower costs. New York seeks to achieve the Triple Aim through a multi-faceted approach based on an advanced primary care model that integrates care with all parts of the health care system, including behavioral health and community-based providers and aligns payment with this care model.
In December 2014, the New York State Department of Health was awarded a $99.9 million Round Two Model Test Award State Innovation Models (SIM) grant by the Centers for Medicare and Medicaid Innovation (CMMI) to implement the State Health Innovation Plan (hereafter "the Plan"). New York developed the Plan and the SIM grant application with the support of numerous stakeholders. The state's official project period of the grant begins February 1, 2015 and will continue for four years.
The Plan is built on a strong foundation of “Advanced Primary Care” (APC), defined as an augmented patient-centered medical home (PCMH) that provides patients with timely, well-organized and integrated care, and enhanced access to teams of providers.
