How are you managing your patient care transitions and preventing rehospitalization? Partnering with community-based providers can ensure success in retaining patients in their homes and communities while ensuring high quality care and health outcomes. Learn more about how home-based care providers improve care transitions in care for PCMH providers at the Visiting Nurse Associations of America's 33rd Annual Meeting in New Orleans, April 20-22, 2015.

On Monday, April 20, an in-depth preconference, "Population Health and the Impact on Care Redesign and New Payment Models," will discuss how new models of health care based on population health must integrate the clinical needs of the patient and family across all sites of care, including the home.

Educational programming on April 21 and 22 explores how providers across the entire health spectrum can work together to transform patient care to meet quality, efficiency, health outcome, and patient satisfaction goals. Participants will gain insights on how to apply new innovations and lessons learned in their own practices, organizations, and partnerships. Highlights include:

Managed Long Term Care, New York State's Solution to Managing the Care of Chronically Ill and Aging Medicaid Recipients
Pulling Back the Curtain: Untapped Areas of ACA Opportunity in your Current Care Programs
