The award-winning Chronic Care Professional (CCP) Health Coach program builds 21st century health care workforce competencies recommended by the Institute of Medicine, and critical patient engagement, lifestyle management, disease management, shared decision-making, self-care, and health behavior change support skills. It is the only nationally recognized health coaching certification, carrying endorsements by the National Care Continuum Alliance, Case Management Society of America, Partners in Improvement Alliance, top-ten patient-centered medical home (PCMH) collaboratives, and the National Association of Disease Management and Wellness Professionals. Over the past ten years, CCP has been linked with improvements in medical home quality and cost outcomes by Kaiser Permanente, regional health care quality outcomes in pilots funded by the states of Minnesota and Wisconsin, and better patient engagement and clinical outcomes in independent evaluation studies.
CCP 5.0 includes four modules: 1) Population Health Improvement; 2) Chronic Conditions; 3) Lifestyle Management; and 4) Health Coaching. CCP features a comprehensive learning and reference manual, online learning center, motivational interviewing (MI) video and skill-building activities—and on-demand replays from the Partners in Improvement Learning Collaborative. Faculty include national subject matter experts from the Mayo Clinic, Cleveland Clinic, Harvard, Johns Hopkins—including trained, credentialed specialists in the only evidence-based approach to health coaching (motivational interviewing) backed by over 300 clinical trials.
CCP is required or recommended by state health units, health plans (including 25 Blue Cross Blue Shield affiliates), many of the nation’s top 100 health systems, the U.S. Air Force, VAs and PCMH groups around the U.S. CCPs are eligible for listing in the national Health Coach Registry: www.healthcoachregistry.org and an advanced certification in MI is also available (Registered Health Coach).
