This program offers a month-long patient-centered medical home team block rotation composed of first, second and third year residents. During this rotation, residents participate in hospital patient searches, multidisciplinary team meetings, PDSA cycles (Plan, Do, Study, Act), and weekly didactics and projects focused on medical home basics, patient safety, group medical visits, documentation, and chart audits and coding. Based on a developmental spiral, each year residents participate in the rotation to further advance their leadership and clinical skills for practice in a medical home environment.
By comparing data from exit interviews of its graduates in 2011 and 2012, this program was shown to have marked improvement in knowledge and skills achieved. It produced residents with more direct medical home experiences (e.g., completing a PDSA cycle, conducting chart audits, participating in and leading group medical visits) and resulted in residents who were better prepared to practice in a medical home (e.g., more prepared to run a group medical visit and conduct a PDSA cycle).
