Lynnzy McIntosh is Executive Director at the Consortium for Older Adult Wellness (COAW). COAW is a Colorado non-profit organization and recognized leader in engaging and inspiring adults, communities and health systems in realizing their prevention, wellness and self-management potential. In addition to administrative duties she supervises integration of falls prevention and self-management curriculums into health systems, community health centers, and individual practices seeking recognition as Patient Centered Medical Homes. She is a part of the COAW team working with software to manage clinical referrals to community-based programs in New Mexico, Maryland,  Illinois, and North Carolina. She co-chairs the National Council on Aging’s workgroup on Community-Clinical Integration and is proud to be a member of the PCPCC Patients, Families, and Consumers Center cabinet.
