In her role with TransforMED as VP, Healthcare Solutions, Diane provides oversight and direction to the TransforMED team as they assist primary care practices in transformation to the Patient Centered Medical Home model of care. Diane works to identify best practices, using data and observations gleaned from the TransforMED work with primary care practices and healthcare systems across the country, to ensure that there is ongoing improvement in transformation resources available to primary care.
Diane leads the TransforMED team to ensure that primary care practices working on PCMH transformation maximize use of available resources. In addition she works with healthcare systems to ensure sustainability and scalability of the transformation process. Her experience includes working with primary care practices, payors, sub-specialty groups, hospital representatives, Beacon Communities, regional extension centers and technology vendors to identify best practice models that expand PCMH care to an effective PCMH neighborhood in a value based reimbursement system. She has supported numerous primary care practices to achieve NCQA Level III PCMH recognition, co-developed a practice coach curriculum, co-developed care manager/care coordinator curriculum, worked directly with hospital and sub-specialty-primary care teams to develop effective medical neighborhood standards. All with a focus on supporting an effective primary care model of patient centered care. Her PCMH practice experiences include small solo practice setting, large integrated system practice settings, federally qualified community health centers, residency practices and academic healthcare settings.
Diane brings to her position and the practices she works with 15 years of primary care clinical experience. In addition Diane has worked in an administrative role in a multi-specialty clinic working with physicians, NP/PA providers and staff. Her experiences include working with solo provider practices in rural settings and primary care departments in a large multi-specialty group. She brings firsthand experience with efficient NP/PA/Physician team care, policy development, open access scheduling and many coordination of care components.
Diane received her MPA with healthcare emphasis from Drake University, ARNP from University of North Dakota and her BSN from Fort Hays State University. She has co-authored a PCMH workbook series on Access to Care; Care Management and Care Coordination.
