Family Health Center

Practice Description: 

Family Health Center provides the full scope of primary care services for children, adolescents, adults, elderly and homebound patients. Services include continuity and urgent care for all ages, prenatal care and perinatal case management, family planning, HIV family clinic, well child care, minor office procedures, mental health services, social services, substance abuse counseling, nutrition consultation, diabetes education and case management, nurse case management, pharmacist consultation, managing patients on anticoagulation therapy, vasectomy clinic, back pain clinic, and psychosocial services.  Special programs for children include a teen clinic and a pediatric oral health screening clinic for children up to 5 years of age.

The Family Health Center operates under the San Francisco Health Network, which believes that patients must be active participants in the decision- making process of the health care they receive. Care that is truly patient-centered considers patients’ cultural traditions, their personal preferences and values, their family situations and their lifestyles. It makes the patient and their loved ones an integral part of the care team who collaborate with health care professionals in making clinical decisions.
