PFCC.Connect is a free on-line learning community bringing together patient and family advisors, health care clinicians, staff, community members and others who are working in partnership to improve health care across the continuum. With over 1500 members and growing, this is the place to learn together, share resources, tools, insights, and experiences to advance the field of patient- and family-centered care.  

 

Special topic groups as well as the at-large community helps connect people with other organizations and individuals who share the same background, roles, and experiences. Dynamic discussion groups on patient family advisory councils, primary and ambulatory care partnerships, research partnerships  and statewide patient and family provide a forum to explore these key topics.
