November month in review: #PCPCC2015 conference recap, TCPI toolkit, new PCMH program results & infographic
Much to be Thankful For... Tuesday, November 24
Dear Members and Friends,
As we celebrate Thanksgiving this week, I cannot help but reflect upon our recent Annual Fall Conference and feel grateful for everyone who helped to make it a success! I want to give a special word of thanks to our patients and families who generously shared their stories with us. We heard honest stories of unbelievable courage, stories of fear, but more than anything, stories of thanks for health professionals who are willing to listen, care, and provide hope. Not one of us left the conference without feeling incredibly moved by the words of 12-year-old, Ayah – seeing the health care system through the experiences of her and her family. We appreciate all the patients and families who are willing to partner with us at the PCPCC as we work together to make improvements in health care for patients and professionals alike.
I am also grateful for the exciting momentum behind advanced primary care, the patient-centered medical home, and health care transformation. Last week, we had an opportunity to respond to the Centers for Medicare and Medicaid Services (CMS) Request for Information (RFI) on Alternative Payment Models and the Merit-Based Incentive Payment System (MIPS). Our comments underscore the critical need for increased investment in primary care, as well as strong support for the entire care team. We invite you to read our response letter, as well as letters submitted by some of our executive members, on our MACRA resource page.
As we count our blessings, I want to thank YOU for your support of the PCPCC and for your commitment to helping advance an effective and efficient health system built on a strong foundation of primary care and the patient-centered medical home. Warm wishes for a Happy Thanksgiving!

Survey participants will be entered into a raffle to win a $50 Visa gift card.
As our CEO Marci Nielsen mentioned in her closing remarks, we are grateful for your support, and we could not do the important work we do without you. If you are not already a PCPCC executive member, we hope you will consider joining us!
The PCPCC, in partnership with its executive members and in collaboration with its Stakeholder Centers, recently developed an infographic to help educate patients, families, and consumers on the value of the PCMH model of care delivery. Access web and print versions of the infographic here!
