A Spooktacular Time for Primary Care

It has been an exciting — or should I say "spooktacular" — Patient-Centered Care Awareness Month at the PCPCC! In late September, the U.S. Department of Health and Human Services (HHS) awarded us one of 10 Support and Alignment Network grants as part of the larger Transforming Clinical Practice Initiative (TCPI). This grant  will allow the PCPCC, in partnership with the Institute for Patient- and Family-Centered Care, Planetree, and YMCA of the USA (Y-USA), to support both clinicians and patients as they learn new strategies for working together to guide quality improvement activities in primary care practices. Join us today for our National Briefing webinar to learn more about this exciting new initiative. 
 
We are thrilled to celebrate all of this new work with our executive members and participants at our upcoming Annual Fall Conference (#PCPCC2015), which is less than two weeks away!  As we enter a new era in primary care with the Medicare Access and CHIP Reauthorization Act (MACRA) and TCPI, we have much to celebrate but a great deal of work ahead to support meaningful practice transformation while driving greater investment in primary care. And, as the U.S. government makes a significant investment in primary care through TCPI, we look forward to helping lead the charge toward greater patient, family, and community engagement and an overall increased investment in primary care by all stakeholders.

National Briefing Today!
The Transforming Clinical Practice Initiative:
An Overview of the PCPCC Support & Alignment Network
TODAY Oct. 29 at 1 p.m. EST

Join us for our October National Briefing webinar to hear the details of the PCPCC's Support and Alignment Network (SAN), part of the federal Transforming Clinical Practice Initiative (TCPI) grant awards, announced by the U.S. Department of Health and Human Services last month. TCPI is one of the largest federal investments designed to support ambulatory care teams in all 50 states through collaborative and peer-based learning networks over the next four years.  

This webinar will provide an overview of the PCPCC's SAN grant, including its four main goals, metrics, and the roles of our partner organizations. Titled "Patient, Caregiver, & Community Engagement Support & Alignment Network (PaCCE SAN)", the PCPCC will collaborate with the Institute for Patient- and Family-Centered Care, Planetree, and the YMCA of the USA to carry out this work.
