This Program empowers primary care physicians (PCPs) to engage in those comprehensive primary care functions that move us toward a coordinated, evidence-based care model that has the greatest impact on achieving the triple aim of improved quality, patient experience and affordability.
The Program was implemented in several counties in Northern California in early 2013, starting with Napa, Sonoma, Shasta and Tehama, and then expanding to additional counties later in the year. The program now includes San Diego, Los Angeles (and surrounding area), Orange, Riverside, San Bernadino, San Luis Obispo, Ventura, Kern, Sacramento, Placer, Yolo, Fresno, Santa Clara, Santa Barbra, Humboldt, San Mateo, Napa, Sonoma, Shasta, and Tehama counties.
