An Overview of the Results
Throughout the month of January, PCPCC programs will focus on the findings from our forthcoming Annual Report (public release scheduled for January 14th). This year the PCPCC Annual Report, supported by the Milbank Memorial Fund, presents a showcase of the year's 20 peer-reviewed and industry-generated studies that indicate the patient-centered medical home's impact on cost, quality, and population health outcomes.
