A Tale of Two Systems: From Medical Home to ACO

We're excited to be hosting our Monthly National Briefing at 11AM today, where we'll feature presentations from two health systems and the lessons they've learned along their journey from medical home to ACO. We'll hear from Michael P. Jeremiah, MD of the Carilion Clinic, a young ACO in Roanoke, VA (presentation slides); and Deborah K. Redmond from UPMC Health Plan in Pittsburgh, PA (presentation slides).

As we well know, the medical home plays a critical role in shaping the structure, success, and future direction of ACOs. Both models share a common vision for a high-performing health system built on a strong foundation of primary care. They are both enhanced by key value-driving elements, such as access, care coordination, health IT, and payment reform. And both hold tremendous promise for achieving the goals of the Triple Aim: better care, better health, and lower costs.

New Health Affairs Study:
The Medical Home's European Appeal
A new study from Health Affairs: Survey Of 5 European Countries suggests that the medical home model could benefit Europe's primary care system, particularly in improving care for the chronically ill.

While certain elements appear to be widespread, such as access to a physician and after hours care, some medical home features were not, such as written self-management support.

National Scorecard on Payment Reform Shows Value-Based Design Still Lagging
 
 
CPR released its first ever National Scorecard and Compendium on Payment Reform. Results show that only about 11 percent of the health care dollars paid are value-oriented, or tied to quality of care, performance, and/or reducing waste. 

The remaining 90 percent of payments are traditional fee-for-service; which pay providers based on quantity of tests, procedures, and visits.

New NASHP Resource Tracks Details of State Innovation Models
The National Academy for State Health Policy State (re)Forum launched a new online chart that provides a comprehensive crosswalk of the six State Innovation Models announced by HHS in February (Arkansas, Maine, Massachusetts, Minnesota, Oregon, and Vermont). The grants provide up to $300 million to support the development and testing of state-based multi-payer and care delivery transformation models. The resource includes a crosswalk of each state's:
