Spreading Better Care, Far and Wyden Thursday, January 30th
Dear Members & Friends:
Late last week, the incoming Chair of the Senate Finance Committee, US Senator Ron Wyden D-Ore., together with three co-sponsors, Sen. Johnny Isakson, R-Ga., and Reps. Peter Welch, D-Vt., and Erik Paulsen, R-Minn. introduced The Better Care, Lower Cost Act of 2014. The bill’s aim is to transform Medicare from a program that pays for services separately to one that pays teams of caregivers for providing individualized, long-term care to chronically ill patients with two or more illnesses. This population currently accounts for 93% — about $487 billion annually — of Medicare spending. The proposed Better Care program removes the barriers that prevent Medicare providers from building on existing successful delivery models – like the patient-centered medical home -- and provides a framework for encouraging innovative chronic care delivery across the country. We will be following the progress of this and other proposals that appear to be gaining traction as we seek better solutions to support individuals with chronic illness – and the providers who treat them -- through patient-centered medical homes.
We also want to share our warm congratulations with Michelle Shaljian, our former Director of Communications, who has accepted the position of Communications Director for the Engelberg Center for Health Care Reform at the Brookings Institution, beginning next week. We are thus looking for a new leader for our communications team. The position description can be found on our website and resumes can be sent to Amy Gibson at agibson@pcpcc.net. In the interim, we are making the shift from weekly to monthly newsletters in order to streamline our communications efforts. Starting in February, newsletters will be sent out once a month on the last Thursday of the month. In addition we will be sending more timely news and policy alerts regarding medical homes and primary care as it happens.

I, along with one of our report co-authors, Nwando Olayiwola, MD, MPH, FAAFP, will discuss key findings of the report and future implications for the medical home marketplace. Earlier this week, Modern Healthcare reported on the The Better Care, Lower Cost Act of 2014 introduced by Senator Ron Wyden (D-OR). The bill identifies a framework for reducing Medicare costs through the creation of payment caps set high enough to fund strategic teams, allowing team members to focus on reducing hospital admissions and keeping overall costs below the cap.
The framework builds on Medicare's Pioneer Accountable Care Organization (ACO) program and incorporates features from patient-centered medical homes, including requiring beneficiaries to receive an individualized care plan.
Patients are at the heart of everything in the PCMH. Through innovations in primary care, we are finding new ways to partner with patients and families in their own care, in quality improvement, and in developing policies that assure patients and families remain at the center of all that we do.
