Dear Members and Friends:
You may have seen a new study released in JAMA, which found that smoother transitions in care effectively lowered readmission and hospitalization rates. This study is one of many that emphasize the power of care coordination in helping patients seek the right care, in the right place, and at the right time.
However, we recognize that building coordinated and integrated systems of care or 'medical neighborhoods' is no easy feat. As you all know, it requires intense practice transformation, new payment models, sophisticated data management, and the adoption of technologies to help patients and providers stay informed and connected.
As you continue on this journey, we look forward to keeping you connected with the resources, experts and educational opportunities you need to build medical neighborhoods. On Wednesday, Jan. 30th we'll host a webinar where employers and providers will share their own perspectives on accountable care; and on Thursday, Jan. 31st our monthly national briefing will highlight the work of PCPCC Executive Committee members Wellpoint and Bon Secours Medical Group, an accountable care model in Richmond, VA that incorporates innovative payment strategies. Call for Submissions: Innovative Residency & Health Professional Training Models We're looking for innovative residency and health professionals training models that promote collaborative, patient-centered primary care. Submissions will be used to build an online database, as well as develop a publication that informs policymakers and other stakeholders.
