PCPCC's 2015 Annual Fall Conference will return to DC this November, and will include dozens of workshops and panel presentations for clinicians, patients, employers, researchers, and advocates. 2015 CAPG Healthcare Conference
Accountable Care: Accelerating the Evolution

This year, the PCPCC is delighted to co-host a preconference session at CAPG's 2015 Healthcare Conference. PCPCC's preconference session, Primary Care Innovation and the Patient-Centered Medical Home, will present innovative primary care strategies organizations are using to improve population health and strengthen patient and community partnerships. The preconference will take place on June 11th from 1 - 4:30 p.m. PT.
