New PCPCC Resource: Primary Care Innovations and PCMH Map
The PCPCC Is Mapping the Medical Home Movement!
Dear Members and Friends:
PCPCC is excited to announce the launch of our Primary Care Innovations and Patient-Centered Medical Home (PCMH) Map! This new online resource captures the extensive work and commitment of nearly 500 public and private initiatives nationwide that are working to improve the U.S. health care system through enhanced primary care and the PCMH.
The map is unique in its breadth, depth and convenience, collecting information on medical home initiatives across multiple programs, payers, and commercial and public health plans. Although this tool does not currently map individual medical home clinics, practices or centers, it allows users to identify where medical homes are becoming a standard of care in commercial and public sector health plans – useful for state and federal policymakers, researchers and educators, health professionals and other stakeholders.
Over the years, we have worked to track and disseminate information about PCMH-related activities across the nation. In fact, one of our first products was an online map to track PCMH demonstration programs. Many of the demonstration programs were expanded to larger programs after successfully lowering costs and improving health outcomes. Our new map reflects the current widespread implementation of PCMH and other enhanced primary care programs across the country. As the number of PCMH programs continues to grow, we will update our map accordingly.
The program pages and state descriptions were populated using information from a variety of publicly available resources. Users are encouraged to follow the links provided on each program page to the original sources of information as well as links to other original documents in the summary descriptions. In an effort to maintain accuracy and the integrity of the map, the PCPCC welcomes edits and additional program information. Please submit edits and/or suggestions to pcmhmap@pcpcc.org.
