As we wrap up our month-long dedication to state innovations in patient-centered medical homes (PCMH), we are thrilled to report on the impressive work happening in states across the country.

First, we are excited to share our new resource that documents state-level PCMH activity and legislative language within state health insurance exchanges. You'll note that there are many more opportunities for states to include PCMHs into their health reform efforts! This chart was developed in collaboration between PCPCC's Advocacy & Public Policy Center and the National Academy for State Health Policy (NASHP). You can access the chart on State Refor(u)m's website here.

For a more detailed look at state-level medical home activity, we encourage you to check out PCPCC's recent webinar, "State Innovations: Updates from the Minnesota Health Care Home Initiative," which featured an in-depth look at Minnesota's medical home program, and included key findings from the program's evaluation report. Finally, we hope you'll join us today for our Monthly National Briefing, "Medical Home Innovations at the State Level." Dr. Richard Snyder, Chief Medical Officer at Independence Blue Cross (IBC), will provide an inside look at the Pennsylvania Chronic Care Initiative, which was found to reduce hospitalizations and medical costs for patients with multiple chronic conditions. Dr. Lisa Dulsky Watkins, Principal, at Granite Shore Consulting and former Deputy Director of the Vermont Blueprint for Health, will share insights on various state multi-payer primary care transformation efforts.

Oregon Leading the Trail to Payment Reform

A consistent innovator in the field of state health reform, Oregon launched the Alternative Payment Methodology (APM) demonstration project as a means to shift the payment structure in Oregon from fee-for-service to one that rewards value over volume. The APM pilot provides community health centers (CHCs) a monthly payment to incentivize a care delivery system that is value-driven, team-based, and patient-centered.

The state will compare the pilot's payment data from March 2013 - March 2014 against the payment that each site would have received under the fee-for-service payment structure. Health Affairs will continue to report on the APM pilot as part of its series on payment and delivery reforms in Oregon and Oklahoma.

VA PACT:
Lessons Learned

This month's Journal of General Internal Medicine includes a supplement with 19 peer reviewed articles, focused on the many lessons learned by the Veteran's Affairs Administration as they roll out the Patient Aligned Care Team (PACT) model to more than 7,000 primary care teams across the nation.

Older Adults Like Medical Home Model

Earlier this month, the John A. Hartford Foundation released results from its public poll, "On Your Team: What Older Adults Think About Team Care and Medical Home Services." The survey found that key components of the medical home, including team-based care, can improve the health of adults over the age of 65. Of the number of adults who reported receiving team-based care, 83% said it improved their health. Of those who are not receiving team-based care, 73% reported they want such care and 61% reported that it would improve their health.

The survey measured responses from 1,107 adults over the age of 65 and has a margin of sampling error of 3.9 percentage points.
