Promising new results from Independence Blue Cross were just released in the Journal of Managed Care showing that patient-centered medical homes can better serve chronically ill patients. With 5 percent of the population accounting for nearly half of all health costs, the PCPCC believes it is important to target those who are determined to be most at-risk, especially for interventions intended to reduce costs. In other news, integrating behavioral health into primary care is this month’s theme, dovetailing with the recent Annals of Family Medicine release of the Joint Principles for Integrating Behavioral Health Care into the PCMH. A longtime supporter of behavioral and mental health integration into primary care, the PCPCC expressed our strong support of this critical component of the PCMH in our September 2013 letter to the Senate Finance Committee. In a forthcoming Annals article, we will also be voicing our support for an increased focus on behavioral and mental health integration into the medical home as part of the recognition and certification process, so stay tuned!

The American Journal of Managed Care just released results from a three-year study of Pennsylvannia's Chronic Care Initiative by Independence Blue Cross (IBC). This study found significant reductions in costs and inpatient medical services among high-risk patients enrolled in patient-centered medical homes; IBC reported total annual cost savings of 7.9% to 11.2% ($75 to $107 PMPM) and 8.6% to 16.6% fewer hospital admissions.
The study compared health care cost and utilization results of nearly 700 high-risk patients enrolled in a medical home against a similar control group between 2009 and 2011.

SGR Repeal Bill Agreement Unlikely to Pass
Modern Healthcare reported yesterday that Congress will not reach an agreement on how to pay for the permanent repeal and replace of Medicare's sustainable growth rate (SGR) before the March 31 deadline. The House will instead pass another temporary 12-month patch.
While the PCPCC is disappointed that a bipartisan agreement for a permanent 'doc fix' could not be reached, we are hopeful that Congress will continue to embrace the policies outlined in the SGR Repeal and Medicare Provider Payment Modernization Act as they continue to work together to repair the flawed US health care payment system.

Sustainable Behavioral Health Integration in 7 Steps
With over 40% of primary care patients suffering from behavioral health conditions, the patient-centered medical home is in an excellent position to provide accessible, value-added behavioral health care to patients across the US. A new study released this month by the Annals of Family Medicine hopes to encourage sustainable collaboration and integration of behavioral health in the medical home.
The Annals study provides seven necessary components to ensure value-added integration, which include: 1) combining benefits; 2) targeting complex patients; 3) using proactive behavioral teams; 4) matching expertise to treatment; 5) defining and measuring outcomes; 6) applying evidence-based treatments and 7) using cross-disciplinary care managers.
Medical home implementation and innovation at the state level plays a role in improving health care quality while reducing costs. Several states are leading the nation in efforts to study and implement integrative primary care, and are showing impressive improvements in cost and quality outcomes. NCQA Released New Medical Home Standards
NCQA released its new Patient-Centered Medical Home Recognition standards on March 24th. Key changes to the standards include the integration of behavioral health, greater emphasis on team-based care, and a focus on high-need populations. Learn more about the new standards here.
