New Leadership, New Energy, Renewed Focus!

Dear Colleagues and Friends: 

Yesterday we announced some exciting news, and, if you haven't already heard, I am thrilled to share it with you! Our Board of Directors has hired a remarkable individual to serve as the next President & CEO of our Collaborative. Ann Greiner, former Vice President at the National Quality Forum, will assume the role on April 3. 

Ann has dedicated her entire career to advancing the quality of U.S. health and health care. She has more than 25 years of experience, including senior-level policy, research, and communications roles at prestigious national not-for-profit organizations (read her bio and access the press release for further details).

Our Board is particularly impressed with Ann's passion for and deep understanding of primary care, its critical importance to the foundation of our health care system, and the imperatives for fostering primary care that is high-performing, patient-centered, and that is delivered seamlessly by a multi-disciplinary team of health care professionals. 
 
As I pass the baton to Ann, I cannot tell you how excited I am about this new chapter for The Collaborative. Ann, along with our new Director of Operations Krista Conley, will lead this organization at a critical time in history. Now more than ever we need a powerful, multi-stakeholder coalition like ours to boldly advocate for advanced primary care.  

Our Collective Power is Great - Join Us!
We very much look forward to the important work ahead. You can expect a stronger focus on policy and advocacy; new, valuable content exclusively for executive members; and greater opportunities for partnership.

If your organization is not already an executive member, I encourage you to join us! Earlier this week we hosted a special conference call town hall meeting exclusively for executive members where we had an insightful, engaging discussion about the new legislative climate. Executive members also had the opportunity to provide feedback on a draft public statement we will soon publish on behalf of the more than 70 executive member organizations we represent.

At this critical time in U.S. health policy, we are in a unique position to effect change. The power of our collective voice can make a difference, and together we not only can, but we will, make a significant impact.
 
As I sign off and say farewell, I leave you in very capable hands. Please join me in extending a warm welcome to Ann!

Practice Transformation Corner

Establishing Strong Person & Family Presence in Your Practice

Recognizing that it will improve overall quality of care, many medical practices commit to involving patients and families in their quality improvement (QI) efforts.

While there is no one way to begin, discussing options with your QI team is a great first step. See where the passion for change exists and then build on it. Those key conversations will provide clarity and may illuminate an effective approach for your organization. It is important to build on the strengths of your organization and what has worked best in the past for making changes.

Some ideas include:
  • Convene a Person and Family Engagement (PFE) Steering Committee comprised of key clinic staff/clinicians who are influential and passionate about PFE. Access our resource materials and recorded webinars  that offer shared learning experiences for this type of committee.
  • Establish a system-wide Person and Family Advisory Council (PFAC). The PFAC would help identify improvement opportunities and provide input about changes that impact the entire system. It can also serve as a pilot/model for other clinics and provide support from system/centralized resources.
  • Integrate patient and family members with lived experience into improvement projects related to that experience. 
  • Join a learning community such as the IPFCC PFAC Network, a forum for clinicians and staff to discuss patient and family engagement from multiple perspectives and obtain and share ideas about how to build robust PFE systems. 
Our Collaborative serves as a Support and Alignment Network through the federal Transforming Clinical Practice Initiative (TCPI), and our focus is to promote effective strategies for person and family engagement. In partnership with the Institute for Patient- and Family-Centered Care, Planetree, and YMCA of the USA, we provide technical assistance to practices and practice transformation networks participating in TCPI. We offer virtual support including coaching calls, onsite training, webinars, and tool development. Contact us for more information!
Collaborative Issues Public Statement Regarding Health Care Reform
As Congress explores changes to the health care system, the Primary Care Collaborative issued a public statement emphasizing the need to continue to advance coverage of primary care and to align payment reform for primary care as essential to achieving better health outcomes and lower costs.

Created in collaboration with executive members, the statement features the following points: 
  • Advanced primary care is foundational to delivery system transformation, and patient-centered medical home initiatives continue to reduce health costs and unnecessary utilization of services (PCMH Evidence Report 2016).
  • Americans must have access to affordable coverage for primary care services, without which the health of individuals, families and their communities will be threatened.
  • Payment reform – innovative alternate payment models – must continue to be advanced, in both the public and private sectors to encourage and support primary care.
