There's never been a more promising time for primary care and the patient-centered medical home (PCMH). As demonstrated in the PCPCC's most recent report, The Patient-Centered Medical Home's Impact on Cost and Quality, Annual Review of Evidence 2013 - 2014, enhanced primary care in the form of the PCMH has the potential to change the quality and cost of health care in America.

As part of our effort to strengthen the medical home movement, we are seeking out new partnerships with organizations who are passionate about advancing an effective health care system built on a strong foundation of primary care. PCPCC Executive Members are a dedicated group of leaders who play a fundamental role in the success of the organization and are dedicated to transforming the U.S. health care system through delivery reform, payment reform and patient engagement.
