Population Health Management in the Medical Neighborhood
We are delighted to share a preview copy of our latest publication with the PCPCC's eHealth Special Interest Group. The publication titled, "Managing Populations, Maximizing Technology: Population Health Management in the Medical Neighborhood." focuses on the critical role of health IT-supported population health management across traditional providers and community organizations in the medical neighborhood. The paper provides a primer for those interested in learning more about the role of health IT tools in gathering, interpreting, and using population health data to improve patient experience and health outcomes.
It also includes a "Top Ten List" of recommended health IT tools that build on the five key attributes of the patient-centered medical home and the medical neighborhood: person-centeredness, access and continuity, coordination, commitment to quality improvement, and comprehensive, team-based care.
The publication will be released to the public and available in hard copy during our Annual Fall Conference October 13 - 15th. The Care Delivery & Integration Stakeholder Center will also host a conference workshop on October 15th that focuses on the paper's findings and recommendations.
