Privately owned Internal Medicine/Family Medicine clinic, comprised of 3 physicians and 3 mid-level providers. All physicians are board certified with one of the Internal Medicine physicians being dual boarded in IM and Endocrinology.
The practice was founded in 2008 and opened the doors using an Electronic Medical Record system. The three physicians were able to attest for Meaningful Use in April of 2011 due to their proactive stance in staying abreast of the multitude of changes within healthcare.
We are a level 1 Patient Centered Medical Home in processes of improving practices to become certified by Joint Commission as a level 3 Patient Centered Medical Home. We are currently a rapidly growing federally funded rural health clinic. We service primarily uninsured or underinsured individuals of all ages. We look forward to becoming part of the collaborative.
Community Recovery Inc has been a transformational organization in addiction treatment bringing together the power of community and family with the individual in crisis. We are now exploring the best way to support the movement towards PCMH and how primary care and behavioral health can best support each other in the paradigm.
Based in Los Angeles, Community Recovery works in both high need and highly affluent communities, recognizing money is not the only barrier to excellent medical care.
