Arizona is focusing its patient-centered medical home (PCMH) efforts on targeted populations including children with special health care needs, dual eligibles, and Native Americans. Beginning October 1, 2013, the Chidlren's Rehabilitative Services program by the Arizona Physicians IPA  will become an integrated health program, offering members both physical and behavioral health services.  As outlined in the Arizona Medicaid Program Strategic Plan, a Tribal Care Coordination Initiative is working to improve health outcomes among Arizona tribal members through the implementation of program initiatives that suppport care coordination and encourage the use of medical homes.

ACOs, other delivery reforms shift job roles at hospitals

Phoenix obstetrician Megan Cheney no longer makes hours of telephone calls on Thursday nights to report routine results of laboratory tests to waiting patients. The calls, however, still get made every week.

Phoenix Children's and Arizona Blues team up to curb obesity

In a rare direct payer-provider partnership, Phoenix Children's Hospital and Blue Cross Blue Shield of Arizona are launching a medical home program this month to curb and prevent childhood obesity. The idea for a jointly operated childhood obesity clinic started about a year ago, said Dr. Kristen Samaddar, a pediatrician at Phoenix Children's and one of the leaders of the year-long program.

To help support professionals in providing integrated care services, we are delivering a series of office-based training sessions. Offerings are targeted for primary care physicians (PCPs), skilled nursing facilities/assisted living facilities and behavioral health professionals.
