Patient participation rates are high

HARRISBURG, PA -- Today marks the one-year anniversary of Capital BlueCross' launch of two Accountable Care Arrangements (ACA). Preliminary results demonstrate that this innovative care delivery model is improving care coordination, lowering the medical cost trend, and improving the member's management of chronic conditions and overall satisfaction with care.  This initiative builds off of the experience Capital BlueCross gained after implementing the region's first private patient-centered medical home pilot in 2010.

Horizon's Early Results Show Patient-Centered Medical Homes Drive Quality and Cost Improvements

Horizon Blue Cross Blue Shield of New Jersey announced “ground-breaking” results from its statewide PCMH program.  Collaborating with leading primary care doctors and the New Jersey Academy of Family Physicians, BCBSNJ developed the PCMH program with the goal of increasing accountability and improving coordination and quality of patient care.  Preliminary quality and cost trends from 2011 indicate that this program reached its goals of improving quality while at the same time reducing health care costs.

Aetna Utilizing Medical Home for its Medicare Advantage Populations

In efforts to improve quality of care and lower health care costs, Aetna and the Connecticut State Medical Society Independent Practice Association (CSMS-IPA) announced on March 13, 2012 in the New York Times that they are designing a program based on the collaborative care model for its Medicare Advantage members.  Nurse case managers and physicians will collaborate in order to improve quality of services and provide better care management to this population. Quality and efficiency measures in various clinical areas will be used to monitor improvements in adherence to best practices and tr

Connecticut to Introduce Medical Home Initiatives

In January 2012, Connecticut introduced a PCMH initiative within their redesign of the Medicaid program HUSKY health. Connecticut’s Medicaid program will provide new payment incentives to health care practices and clinics demonstrating higher standards of patient-centered medical care.Additional state support will be provided to practices that currently are not recognized by NCQA as meeting Level 2 or Level 3 status for a PCMH.
