What are the challenges related to engaging multiple payers? What strategies have been used to overcome them? Join AcademyHealth for a Web event hosted by the Agency for Healthcare Research and Quality (AHRQ) Health Care Innovations Exchange to learn about Maryland's Multi-Payer Program, a statewide initiative that supports the development of patient-centered medical homes with financial incentive structures designed to promote provider participation.
AcademyHealth and Westat are pleased to work with the AHRQ Health Care Innovations Exchange for this Web event.
With new care delivery models moving ahead in the market and encouraged in the Affordable Care Act, understanding the results will shape policy moving forward. While core elements of these models vary - i.e., primary care logistics, team-based care, patient-centered focus, populations' needs - so do the payment mechanisms being deployed to support these models.
New care models and reimbursement schemes present new challenges for health care providers to manage patient populations. This presentation will share strategies for improving clinical outcomes with data, showcasing PCMH and ACO, and will feature Robert Fortini. Fortini is the Chief Clinical Officer for Bon Secours Medical Group in Richmond Virginia.
To highlight the release of a new Alliance of Community Health Plans (ACHP) publication on primary care, “Health Plan Innovations in Patient-Centered Care: Strengthening Primary Care for Patients,” ACHP is gathering a panel of health care and delivery system transformation experts who will discuss health plan and community efforts to implement patient-centered reforms and improve the care experience for both patients and providers.
This webinar will provide participants with a clear description of family-centered care and highlight case studies that showcase effective strategies in delivering that care through practice improvements such as greater use of health information technology, enhanced access to medical professional staff outside regular office hours, feedback mechanisms capturing patient/family experience with care, and including families on advisory boards and as staff.
Understanding cultural issues is an important component of providing quality health care in the medical home as the US ethnic and racial make-up is changing. Faculty for this webinar will discuss how to provide culturally competent care to patients and families, especially those with limited English proficiency or from immigrant families. Promising practices in the delivery of language access services will also be highlighted.
