To highlight the release of a new Alliance of Community Health Plans (ACHP) publication on primary care, “Health Plan Innovations in Patient-Centered Care: Strengthening Primary Care for Patients,” ACHP is gathering a panel of health care and delivery system transformation experts who will discuss health plan and community efforts to implement patient-centered reforms and improve the care experience for both patients and providers.
ACHP’s publication showcases the work of its member organizations to ensure that primary care transformation is successful, sustainable and scalable; all 17 organizations profiled in the paper (listed below) increased care quality, improved patient and/or provider experience or lowered costs as a result of their targeted work to transform primary care.
The in-person event, which will be co-hosted by ACHP and Kaiser Permanente, will take place in Washington, D.C., on April 9, 2013 from 9:00 – 10:30 a.m. at the Kaiser Permanente Center for Total Health at 700 Second Street NE. The event will feature panelists from various ACHP member plans, including Independent Health, Kaiser Permanente and Presbyterian Health Plan, as well Marci Nielsen, the chief executive officer of the Patient Centered Primary Care Collaborative.
