The 2018 Texas Primary Care and Health Home Summit will be the sixth annual statewide conference focused specifically on expanding access to high quality, person-centered primary care for individuals and families in Texas. This Summit will offer stakeholders the opportunity to learn about innovative primary care models including the health home.
