Kansas
Kansas Statute defines medical home and calls for the Department of Health and Environment to incorporate the use of medical homes within the Medicaid program, the State Children's Health Insurance Program (SCHIP), and the Kansas state employees health care commission. In January 2011, a private collaboration of physicians and foundations created the Kansas Patient Centered Medical Home (PCMH) Initiative. The Kansas PCMH Initiative was intended to advance the PCMH health care model in Kansas, provide education about the PCMH, and provide facilitation for eight practices to pilot transformation to become medical homes. Organizations participating in this initiative were the Kansas Academy of Family Physicians, Kansas Association of Osteopathic Medicine, Kansas Chapter of the American Academy of Pediatrics, Kansas Chapter of the American College of Physicians, and the Kansas Medical Society. In April of 2011, Blue Cross Blue Shield of Kansas pledged payer support for the eight selected pilots. Major funding for the initiative was provided by the United Methodist Health Ministry Fund, the Sunflower Foundation, the Kansas Health Foundation and a Kansas Department of Health and Environment Systems in Sync Grant. The program officially ended in 2013. Currently the Kansas State Medicaid Program, KanCare, is working to launch a Health Home Program in July 2014, pending CMS approval of their State Plan Amendment (SPA).
