The Hopkins Center for Health Disparities Solutions (HCHDS) was established in October 2002 by a five-year grant from the National Center for Minority Health and Health Disparities (NCMHD), of the National Institutes of Health under the Centers of Excellence in Partnerships for Community Outreach, Research on Health Disparities, and Training Program (Project EXPORT).
Manchester Business School are involved in a national collaborative between academic institutions and primary care in the UK to improve the uptake of evidence based practice. My research project is exploring why the care provided for people with multiple chronic conditions is fragmented and suboptimal, with particular reference to the role of physicians in primary care.
Your work is extremely pertinent to my research as many of the issues and challenges are the same in both countries.
